New Patient Instructions

Welcome and thank you for choosing Martin’s Point for your health care!

Below you’ll find instructions for next steps you can take to ensure the best experience for your first visit with your new Martin’s Point provider. If you have any questions, please call your Martin’s Point Health Care Center and a patient services representative will be happy to assist you.


Once you have registered to become a patient and registered for the patient portal, you can complete required forms online via the MyMartinsPoint® patient portal or download and print the forms from this page and bring them to your first appointment.

1. CREATE your MyMartinsPoint® patient portal account:
If you have not yet registered for the MyMartinsPoint® patient portal, click on one of the links below to learn more or register now:

2. COMPLETE the required forms:

(Online Option Using the Patient Portal):

Register and log into the MyMartinsPoint® patient portal to access the forms you need to complete before your first appointment.

  1. In the My Health section on the left side of the MyMartinPoint®home page, select “Medical Forms.” Please complete the first two forms in the list.

  2. Martin’s Point Health Care General Consent: Please open, read, and electronically sign this form. (The electronic signature option is at the bottom of the form.)

    Consent – Medical Release of Information: Please print, fill out, and send this form to your previous health care providers so your medical records will be sent to us.

  3. In the My Profile section on the left side of the MyMartinsPoint home page, select “My Notifications.” Please select your preferences for how you would like to receive Health Notifications, Appointment Reminders, Updates and Announcements, and Billing. 

  4. Two days before your first appointment you will receive a notification that your Health History Questionnaire is now available for you to complete online through the patient portal. 

(Download and Print Option):

  1. Find the General Consent, Health History, and Financial Agreement forms in “New Patient Forms” box to the right on this page. Simply download, fill out the forms and print. Bring the completed forms to your first appointment.

  2. Find the Authorization to Release Protected Health Information to MPHC form in the “Request Medical Records” box on the right of this page. Please print, fill out , and send this form to your previous health care providers so your medical records will be sent to us.
3. NOTIFY your health insurance company of your primary care provider (PCP) change to avoid any delays in billing and authorizations for referrals or prescriptions.

4. BRING the following to your first appointment:
  • Completed General Consent, Financial Agreement, and Health History forms (if you did not submit online)
  • Insurance card and copayment
  • Your medication bottles and a list of any over-the-counter medications you take
  • A list of your health questions/concerns
